Appointments for Minors
If parents/guardians are divorced/legally separated, we require a copy of the legal documents indicating the custody agreement and who has the legal right to consent to medical treatment. In cases of joint custody, we reserve the right to require the signature of both parents before the first session, unless your custody agreement specifics otherwise. ​​​
Insurance
Center for Personal Wellness is an insurance friendly practice and can help clients utilize both their in-network and out-of-network insurance.
In-Network Benefits: For clients using one of the insurances below, we accept the contracted rate for that insurance. We are happy to help you verify your benefits and will file claims on your behalf.

Soon to be covered: We are expanding our network of accepted insurance providers. The following plans will soon be covered for medication management services.​​

Out of Network Benefits: We understand the importance of flexibility in accessing mental health services. Your counseling services may be eligible for reimbursement through out-of-network benefits and for clients with out-of-network insurance coverage, we are pleased to offer out-of-network billing as a courtesy.
Here’s how it works:

Full Payment at Time of Service: Clients are responsible for paying the full cost of each session at the time of service.

Insurance Reimbursement: Upon request, we can either send claims on your behalf or provide you with a detailed invoice, or “superbill,” which you can submit to your insurance provider for reimbursement.

Reimbursement Goes Directly to You: Any reimbursement for out-of-network services will be sent directly to you by your insurance provider.
While we are happy to assist with submitting claims on your behalf, please note that all communication regarding reimbursement will occur between you and your insurance company.

Medication Management: an Investment in Your Well-Being

Your mental health is essential to your overall well-being, relationships, and daily life. Medication management can be a valuable tool in your care, helping to restore balance and support your mental health journey.​

Payment
Payments are due at the time of service and are made at the beginning of each session, including any insurance copayments and/or deductible amounts.
The Center for Personal Wellness is able to accept the following forms of payment: cash, checks, all major credit cards, HSA (Health Savings Accounts) and FSA (Flexible Spending Account) cards.
How often will I come?
Consistency brings the best results.
To ensure effective treatment, we schedule your first intake appointment, followed by two follow-up appointments spaced one month apart. This structured approach allows for proper assessment, monitoring, and any necessary adjustments. However, your provider may modify this plan based on your individual needs.

Over time, your medication management schedule may shift based on progress and stability. Some clients need more frequent follow-ups early on, while others transition to less frequent check-ins as things improve. Your provider will work with you to determine the best plan for ongoing care.

Cancellation + Late Arrival Policy
We value your time and aim to offer quality care. When appointments are canceled or missed, it impacts your ability to receive timely support and limits availability for other clients.
​
Late Arrival Policy: If you arrive more than 10 minutes late, the appointment will be considered a late cancellation and will need to be rescheduled. The cancellation fee will apply, and you will be charged the $100 late cancellation fee.

Cancellation & No-Show Policy:

Cancellations made less than 48 hours of your scheduled appointment will incur a $100 fee.

Cancellations made less than 24 hours or no-shows will incur the full session fee.
​Please note that insurance does not cover late cancellations or no-shows, so the client will be responsible for these fees. The credit card on file will be charged for any missed or late-cancelled appointments.

What's Next?
​
CPW is a paperless practice. After scheduling your appointment, our client care coordinator will set you up in our online portal.
​
Through our secure and HIPAA compliant portal, you will complete all of your intake paperwork and have access to billing. This must be completed 48 hours after scheduling your first appointment. This will allow your therapist to spend more time with you and less on paperwork and will additionally allow us to verify your insurance benefits if you are choosing to utilize them.
Please fill out the intake forms at your earliest appointment. Your appointment time will not be final until your intake paperwork is completed. Sessions cannot start without intake forms being completed.​
